Adult clients have the right to make a declaration of mental health treatment. The declaration of mental health treatment allows individuals to make a plan for a time in the future when they may not be able to make decisions around their mental health treatment themselves, such as in a time of crisis.
If you wish to make a declaration of mental health treatment please discuss this with your primary therapist. Your therapist will provide you with the most recent version of the Oregon Health Authority’s (OHA) A Guide to Oregon’s Declaration for Mental Health Treatment. This is a voluntary process and is not a requirement of receiving services at Cascade Counseling and Consulting.
Upon request, your therapist can support you while you complete the declaration of mental health form, but it is important to understand that we cannot provide legal advice. Should you require legal advice please discuss this matter with legal counsel.
